I'm pretty sure the thigh bands and the bands on the biceps that are visible on the cardback were the original attachments intended by the 4HM. The ones Mattel's design team went with in the end do...

The tails are part of his "hair" and not part of the snakes coming out of his eyes.
The vintage figure had those "hair tips" aswell.

I carefully removed the tails from the regular head (hot water and a small flat screwdriver).
They had to be shortened and cut to fit the gap between the red snakes and the helmet of the alternate...
